Dr. David Bazzo Inaugural Physical Day May 1, 2026 Dr. David Bazzo Inaugural Physical Day May 1, 2026 San Diego Unified student-athletes are invited to attend the Dr. David Bazzo Inaugural Physical Day, a community event offering $10 pre-participation sports exams for students planning to participate in California Interscholastic Federation CIF-sanctioned sports during the 2026–27 school year. Screenings will include the elements required for student-athletes to participate in school sports. Services will be provided by licensed medical professionals. This inaugural event is held in honor of the memory of Dr. David Bazzo and his dedication to student health and wellness. Families are encouraged to join us for a day focused on care, prevention, and community. Event Details Date: Saturday, June 13, 2026 Time: 8:00 a.m. – 12:00 p.m. Location: Canyon Hills High School 5156 Santo Road San Diego, CA 92124 The event will be offered on a first-come, first-served basis. Every effort will be made to see as many student-athletes as possible. What to Bring Student-athletes should bring the following items: * School identification card * Completed San Diego Unified Health History Form, signed by a parent or guardian * Corrective eyewear, if needed * $10 cash payment for the optional screening fee The San Diego Unified Health History Form will be available beginning Monday, June 8 at homecampus.com/login. Check-In Information Students should arrive dressed in athletic clothing and park in the Canyon Hills High School parking lot off Santo Road. Volunteers will be available to direct participants to the check-in area. Please plan ahead for extended wait times. Total visit time may be up to two hours. Parent and Guardian Information Parents and guardians who are unable to attend should complete and sign all required forms and send them with their student-athlete. Payment The optional screening fee is $10, cash only. The screening is available to students participating in CIF-sanctioned sports. Important Notice Completion of a physical exam does not guarantee medical clearance for athletic participation. Student-athletes may be referred for further medical evaluation if needed.
